Please Read Carefully: By booking a session or purchasing a program through RootFix Performance, you agree to these Terms of Service. These terms govern your relationship with RootFix Performance and contain important information about our services, payment, cancellation, and liability.
Root Fix Performance, LLC is a private-pay injury resilience and performance coaching practice located at 8801 University Ave, Suite 28, Clive, Iowa 50235, operating under the brand name RootFix Performance.
Services are provided by Almin Murgic, a Certified Athletic Trainer (ATC), Licensed Athletic Trainer (LAT), and Pain-Free Performance Specialist (PPSC), operating in the capacity of a performance coach and injury resilience specialist.
RootFix Performance is a private-pay practice and does not accept insurance of any kind.

All services are delivered as performance coaching. RootFix Performance does not provide medical services, physical therapy, or any other licensed healthcare service.
Important: RootFix Performance is not a medical practice, physical therapy clinic, or healthcare provider. Our services are performance coaching services and do not constitute medical advice or medical care of any kind.
Nothing communicated by RootFix Performance — verbally, in writing, or through any program or session — should be interpreted as medical advice. If you have a medical condition, injury, or concern, you should consult a qualified healthcare provider before beginning any coaching program.
Participation in RootFix Performance services does not create a patient-provider relationship of any kind.
All new 1:1 coaching clients are required to complete a Performance Assessment before beginning any coaching program. The Performance Assessment is a 60-minute session that:
The Performance Assessment fee is $200. This fee is non-refundable once the session has been completed. Online program purchases do not require a Performance Assessment.
We reserve the right to decline services to any individual whose needs or goals fall outside the scope of our coaching programs.
RootFix Performance does not accept health insurance, Medicare, or Medicaid. All services must be paid for directly by the client. A valid credit or debit card is required on file to book and maintain appointments.
Current pricing is provided at the time of booking through our Jane App booking platform or on our website. Pricing is subject to change with reasonable notice to existing clients.
Coaching packages are non-refundable but are transferable to other services within RootFix Performance at our discretion. Unused package sessions expire 12 months from the date of purchase unless otherwise agreed in writing.
Online program purchases are non-refundable once access has been granted. If you experience a technical issue preventing access to your program, contact us within 7 days of purchase and we will work to resolve it.
Clients enrolled in recurring coaching agreements must provide 30 days written notice to terminate the agreement. Notice must be submitted in writing to almin@rootfixperformance.com.
If a payment fails or a card on file is declined, services may be paused until payment is resolved. Outstanding balances are due immediately upon notification.
24-Hour Cancellation Required: We require at least 24 hours notice for all appointment cancellations or rescheduling requests.
Cancellations made with less than 24 hours notice will be charged the full session fee to the card on file.
Failure to attend a scheduled appointment without prior notice will result in a charge of the full session fee to the card on file.
Clients arriving more than 15 minutes late will be treated as a no-show and charged the full session fee. Sessions will end at their originally scheduled time regardless of arrival time.
We understand emergencies happen. Fees may be waived at our sole discretion for genuine emergencies or unforeseen circumstances. Please contact us as soon as possible if you are unable to attend.
Clients who repeatedly cancel or no-show may be required to prepay for sessions or may have services discontinued at our discretion.
By using RootFix Performance services, you agree to:
To the fullest extent permitted by Iowa law, Root Fix Performance, LLC and its staff shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services.
Our total liability to you for any claim arising out of or related to our services shall not exceed the total amount paid by you to Root Fix Performance, LLC in the 90 days preceding the claim.
Physical activity and performance coaching carry inherent risks including but not limited to muscle soreness, strain, or injury. By participating in RootFix Performance services you acknowledge and accept these inherent risks as outlined in the liability waiver signed at intake.
Liability Waiver Required: All clients are required to sign a separate Liability Waiver and Release as part of the intake process. That document contains additional important terms regarding assumption of risk and release of liability.
RootFix Performance makes no guarantee of specific outcomes, results, or timelines. Individual results vary based on a wide range of factors including training history, program compliance, overall health, age, and lifestyle factors outside our control.
Client testimonials shared on our website or marketing materials reflect individual experiences and are not a guarantee that you will achieve the same results.
RootFix Performance reserves the right to terminate or decline services at any time for any of the following reasons:
Where appropriate, we will provide referrals to other providers when discontinuing services.
These Terms of Service are governed by the laws of the State of Iowa. Any disputes arising from these terms or your use of Root Fix Performance, LLC services shall be resolved in the courts of Polk County, Iowa.
If any provision of these terms is found to be unenforceable, the remaining provisions will continue in full force and effect.
We reserve the right to update these Terms of Service at any time. Continued use of our services after changes are posted constitutes acceptance of the updated terms. Material changes will be communicated via email to active clients.
